Community Residential

  • Adults live in a supportive environment as fully participating members of the community. The homes are established to facilitate individual choice, foster greater independence and community involvement, and provide a comfortable, life-enriching home life.
  • FSI currently has 9 community homes staffed with trained Direct Support Professionals that support those with intellectual disabilities with reaching their desired goals while maintaining a person-center approach to care.
  • Each community home supports 2-4 persons and provides multiple levels of care.
  • Professionals offer assistance with medications, personal care, coordination of medical appointments, specialized medical care, while ensuring safety and promoting a healthy life.

Lifesharing

  • Lifesharing is recognized as both a close personal relationship and a place to live.
  • Lifeshares and individuals are carefully matched and supported by quality professionals to achieve the person’s goals.
  • Adults reside in a family-based setting centered on compassionate care and independence in daily routines.
  • Each person shares life experiences with the supportive family who form a caring household.

Behavioral Support

  • Behavior support provides specialized interventions for adults with intellectual disabilities and autism that engage in challenging behaviors.
  • This service includes an assessment of behavioral concerns and implementation of an individualized behavioral support plan that is focused on the person’s needs while taking into consideration medical, psychiatric, social, communication and sensory issues that create obstacles to successful navigation of the person’s home and community environments.
  • Behavior support focuses on positive approaches along with the least restrictive techniques to offer the highest quality of life for the individual.

Behavior support is based on several philosophical premises: Applied behavior analysis, person-centered planning, self-determinations, and inclusion.
